We will continue to monitor the status of Covid-19 in our area and may require that everyone who participates answer a Covid-19 Virus Screening questionnaire prior to their event. Standard questions include, exhibiting Covid-19 symptoms, recent virus exposure, etc. If required, this printable document will be emailed to you prior to the event and everyone in your party must complete the screening questions before they can participate. I/we agree to follow the neighborhood rules and guidelines set forth by Youthfront, including maintaining Covid-19 safety protocol. I/we will wear mask(s) inside at all-times as well as, use social distancing at all times. I/we understand that all persons listed above will be expected to follow the safety guidelines enforced by Youthfront staff and if anyone repeatedly refuses to follow these guidelines, I understand I/we may be dismissed from serving or participating.

I/we give permission to Youthfront to use any photographs or videos taken for promotional purposes.

I/we agree to hold harmless and release Youthfront Inc., its directors, officers, employees, volunteers and agents from liability for any fault, mistake, negligence, or omission causing damage, loss, injury, or death to me or my child (hereinafter referred to jointly as Damage) arising from my or my child's attendance and participation in any of the YFN experiences, exposure to the Covid-19 virus, including any Damage arising from the provision of emergency medical treatment.

I/we accept the risks associated with the Covid-19 virus pandemic and will be responsible for myself and/or my group to enforce reasonable social distancing guidelines and mask wearing. I will contact Youthfront immediately if anyone in our party shows symptoms or tests positive for Covid-19 within 10 days after leaving.
