Agreement to Follow Safety Guidelines and Policies

My signature below acknowledges that I understand that the participant will be expected to follow all Youthfront Neighborhood safety guidelines and policies. If the Participant repeatedly refuses to follow these guidelines I understand that the Participant may be dismissed from the serve day.

Hold Harmless and Release

I agree to hold harmless and release Youthfront, its directors, officers, employees, volunteers and agents from liability for any fault, mistake, negligence, or omission causing damage, loss, injury, or death to me or my child (hereinafter referred to jointly as Damage) arising from my or my child’s attendance at Youthfront Neighborhood Serve Day, including any Damage arising from the provision of emergency medical treatment.

Use of Likeness

I give my permission to Youthfront to use any oral or written comments made by, and any photographs or videos taken of, the Participant for promotional purposes.
